The doctor, however, is one of those rich fatcats who’d rather sit at his big fancy table eating his boiled goose than help out poor Richard. El Socorro is too far away, and he doesn’t want to ride out there in the rain. (Ironically, “Socorro” means “aid”.) He finally offers to give Richard the medicine he needs, until he realizes how poor the man is. Man, this is the health care reform episode! They scuffle, and Richard accidentally kills him. And the sound guys did a great job with finding the most unsettling “fatal head injury” sound effect that they could. That was the proverbial sickening thud right there. Richard panics and leaves with the medicine.
